According to Hindu legend, Vindya, the god of the Vindhya mountain range, was worshipping Lord Shiva to atone for his sins. He made a special geometric design and created a lingam (a representation of Shiva) from sand and clay. Lord Shiva was happy with his devotion and appeared in two forms: Omkareshwar and Amaleswara. Because the mud mound looked like the symbol 'Om,' the island came to be known as Omkareshwar.
Another story involves King Mandhata from the Ikshvaku dynasty, who is an ancestor of Lord Ram. He worshipped Lord Shiva at this location until the Lord appeared as a Jyotirlinga. It's also said that Mandhata's sons, Ambarish and Muchukunda, performed intense penance to please Lord Shiva, which is why the mountain is named after Mandhata.
